Young at Heart Adult Day Health Care Center is a community-based adult day health care program committed to serving the elderly and those with physical or mental impairments seeking an alternative to nursing homes.

Young at Heart is licensed by the State of California as an Adult Day Health Care Facility and we provide innovative therapies and programs not available at any other adult day centers in the Valley.  Patterned after the “Day Hospitals” in Europe, Young at Heart clients participate in activities designed to improve their situation and quality of life.

Following an initial assessment by a team of nurses, social workers, occupational and physical therapists, an individual care plan is developed to help the participant maintain or improve physical and mental capabilities.

We provide hot meals, transportation to and from the center and activities to help those seeking relief from loneliness and depression. 

Young at Heart also provides professional support for the at-home caregiver, including seminars and classes in nutrition, medication and caregiver respite. 

Working together, we are able to help families and their loved ones live life to its full potential. 

How much does it cost?
The cost of the program is less than in-home care. Medi-Cal and some private insurance companies will cover the costs. 

Am I eligible? 
You may be eligible for the services provided by Young at Heart ADHC... 

  • If you have a medical condition for which your physician has prescribed treatment or rehabilitation services
  • If you require assistance with activities of daily living
  • If you are potentially at risk of requiring institutionalization
